Trouble scanning wristbands

I'm hoping that someone can help me out with this question. Our hospital is currently having a problem with Accu Check scanners reading the barcodes on patient wristbands. As a result, nursing is using work-arounds such as placing patient stickers on index cards. Unfortunately, they often scan the wrong patient sticker which places the blood glucose result on the wrong patient and sometimes resulting in the wrong patient either receiving insulin and not being diabetic or receiving too much insulin because they were already on sliding scale insulin and received insulin when they shouldn't have or received too much.

The people looking into this were considering changing to a vertical barcode and getting printers for each unit. I have read on list-serves and in the literature that only admitting should have access to printing patient identification because of the potential problems. My suggestion was that if the hospital was going to look at going this route that they should look at RFID patient identification instead. Therefore, I would like to pose some questions.
1. Is anyone using RFID for patient identification?
2. Are there scanners that both recognize RFID and barcodes so a nurse doesn't have to carry around more than one device?
3. Would my suggestion work with Accu Check and is anyone already doing this?
4. We are a Meditech hospital. Is anyone using RFID in this manner with Meditech and BMV?
5. Is anyone using RFID for patient identification and a combination of RFID and barcodes for medications? If so, what pharmacy software are you using?
6. Any other thoughts or comments where this technology is?
