Meditech barcode scanning problems

We have been having trouble with bar codes disappearing from the dictionary
and this weekend we found a barcode that was attached to 3 drugs and I can't
figure out how that happened. We are trying to track down what happened.
Anyone found a way to track who, what , when and where a barcode is scanned?
